Job summary
The Operational Lead position will play a key role in providing expert operational leadership in conjunction with the Associate Director and partner organisations across all pathways in delivering on safe and timely discharges, ensuring quality of care.
The post holder will be accountable for the provision of effective professional leadership and pro- active resource management for all relevant pathways. his includes the Transfer of Care Hub (ToCH) Virtual Wards (VW) and Community Bed Based Care (CBBC).
As a professional practitioner, the post holder will also be expected to act as an advocate for Community services, and CSH in particular. To work with the Associate Directors to create an environment of continuous quality improvement and professional development ensuring that the delivery of care is regularly reviewed and that service developments are implemented when necessary.
Responsibilities will also include contribution to the delivery of the key performance indicators and all key strategic activities and developments. This will be supported by high quality communications with all groups of staff both within CSH and across external partners.
Main duties of the job
- Develop detailed and complex technical understanding of discharge to assess pathways and processes to lead service delivery from a point of professional expertise and competence.
- Work with the COO and ADs in providing assurance, safe, and effective service delivery, including responsibility for data reporting.
- To deputise for the Neighbourhood Health Professional lead where necessary including attendance at required system or Alliance forums.
- Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity.
- Develop high quality team working, implementing target setting where appropriate by agreeing standards within the discharge-focussed multi-disciplinary teams.
- Develop and implement systems for audit, quality assurance and outcome measurements.
- Performance manages direct reports and enables personal and professional development which underpins the strategic direction of the service, CSH and Alliance.
- Working with the clinical teams to ensure all activities are met within the agreed financial framework.
- Pro-actively challenge if poor practice is demonstrated by a staff member and ensure appropriate measures are taken expediently to deal with it, with demonstrable consequences.
