You will manage a varied caseload, including complex cases, and assess risk, recommend adaptations, and source bespoke equipment solutions. You’ll also play a key role in safeguarding, moving and handling assessments, and supporting carers.
You’ll work closely with health and social care professionals, always keeping the person at the centre of your practice.
We’re looking for someone who is HCPC registered, with at least two years’ post-qualification experience and a strong understanding of relevant legislation. You’ll be confident in managing risk, leading on complex cases, and contributing to service development.
