Health and Wellbeing Practitioner- HMP Send (part time) – Surrey

The Forward Trust is seeking a compassionate and motivated Health & Wellbeing Practitioner to join our team at HMP Send, Surrey, working 18.75 hours per week. As part of our integrated Health & Wellbeing Service, you’ll support women experiencing substance misuse and low-level mental health challenges, helping them build healthier, more positive futures within a trauma-informed and gender-responsive environment.
Working as part of a multi-disciplinary team, you’ll deliver recovery-focused interventions, provide practical support, and help service users prepare for successful reintegration into the community.
What you’ll be doing:
- Managing a caseload and coordinating individual support plans
- Conducting assessments and developing person-centred care plans
- Delivering structured one-to-one sessions and facilitating group programmes
- Providing harm reduction advice and guidance
- Supporting release planning and through-care arrangements
- Working closely with healthcare, mental health, and prison colleagues
- Maintaining accurate records and contributing to service performance targets
- Undertaking second signatory duties and oral swab testing where required
What we’re looking for:
- Experience supporting individuals with substance misuse, mental health, or complex needs
- Excellent communication and relationship-building skills
- A proactive, organised, and solution-focused approach
- Confidence working within a multi-disciplinary environment
- Strong IT and record-keeping skills
- Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP)

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
