The Mindworks Hub Clinical Practitioner (Social Worker, Nurse or Occupational Therapist) will be a member of the multi-disciplinary specialist team, providing services to children and young people 5-18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems.

The main responsibilities of the post holder will be providing assessments and treatment, either individually or with a member of the multidisciplinary team depending on the complexity of the referral, within both clinic and community settings.  The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway the young person is on.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be supporting the team in leading with the implementation and embedding of the THRIVE Framework across the newly created Mindworks CYPS in North Surrey.

You will have demonstrable expertise working with young people under the age of 18 years.

Main duties of the job

Carry own caseload and undertake holistic assessments including risk of the child/adolescent/family needs, either individually or in identified complex cases jointly with another member of the CYPS Mindworks.
Provide short term treatment to CYP. 
To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.
Act as a lead professional for CYP following initial assessment. Developing an appropriate plan of care.
Offer support for Students on placement
Co-work with other members of the team MDT.
 Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.
 Provide statistical information on all activities undertaken, in line with Trust procedures. Have basic IT skills and Data entry skills to enter clinical information on the database.
Contribute to the team’s overall standard of services provided.
Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.
Through the appraisal and supervision systems, we identify own training needs and undertake continuous professional development.
Ensure high standards of care are provided, and that of dignity, individuality, self-esteem and wellbeing of individuals and their families are given.  Monitored and evaluated on a continuous basis.

Person specification

Qualifications

Essential criteria
  • Degree in Nursing, Occupational Therapy or Social Work
  • Relevant registration with NMC, SWE or HCPC
  • Post graduate qualification in child development, attachment, or family functioning or equivalent experience
Desirable criteria
  • Post graduate qualification to degree level

Experience

Essential criteria
  • Sound experience of working with CYP with a variety of mental health issues
  • Expertise and experience in supervision and supporting the growth of other staff.
  • Proven ability to provide training and education regarding mental health and complex clinical topics
  • Demonstrable expertise in complex assessment of people with both mental and physical health issues
  • Demonstrable ability to develop practical innovations and approaches to issues and complex problems.
  • Awareness of National agendas for the development and growth of CYPS
