Outreach Liaison Worker – Southend on Sea

£12,000 – £12,750 depending on experience

We are looking for a compassionate and skilled Specialist Support Worker to join our integrated healthcare team, supporting adults aged 18+ who are experiencing challenges with drug and alcohol use. This role is based in Southend-on-Sea and focuses on providing tailored, trauma-informed support to individuals residing in supported accommodation, hostels, and B&Bs.
Key Responsibilities:
- Deliver personalised, flexible support to a reduced caseload of service users with complex needs and barriers to engagement.
- Work within a trauma-informed framework to build trust and promote recovery.
- Liaise with housing support workers, primary care services, and other partners to coordinate care and improve outcomes.
- Contribute to service targets and outcome measurements in partnership with Forward and local commissioners.
What We’re Looking For:
- Experience working with vulnerable adults, particularly those at risk of rough sleeping or with substance misuse issues.
- A strong understanding of trauma-informed care and the challenges faced by individuals with complex needs.
- Excellent communication and partnership-building skills.
- Flexibility to work early mornings, evenings, weekends, and travel to other local projects when needed.
