About the role

Our Reablement Support Officers work with a number of people in the community each day. You’ll travel to the people’s homes to support them with regaining skills and confidence to enable them to become more independent, actively promoting their choice and dignity.

It’s an incredibly rewarding role, every day and every visit is different! You’ll work with autonomy and independence but within a close-knit team, and you’ll have regular support from the Reablement Team Manager.

You’ll also work closely with other our health partners such as the intermediate care team.

Part time shifts available rotation (earlies and lates).

16 hours: 7am-11:30am and 6pm-10:30pm (not flexible).

31.5 hours: 7am-4pm and 1:30pm-10:30pm (not flexible)

At £13.74 per hour. 

Your office base will be Chedworth Close in Ecton Brook, Northampton. We pay for whole shifts (not just individual visits) and pay 45p per mile.

What will you be doing?

Day to day, you’ll see multiple individuals in their own homes, you’ll sometimes visit with a colleague but mostly you’ll work on your own.

You’ll then visit them regularly to monitor progress and support them with day-to-day activities which could include personal care, meal preparation, mobility and accessing the community with the aim of enabling them to regain their independence. 

You’ll case manage reablement journeys, from initial assessment through to discharge. You’ll meet people after referral to assess their needs and support each individual to and support in the planning of their reablement goals. You’ll also support people by signposting them to other agencies where appropriate, for example Assistive Technology, Age UK, Northamptonshire Carers etc.

About you

  • You’ll be confident working independently and have the ability to handle unexpected situations or challenges, being able to identify the source of a problem and find an effective solution.
  • You’ll need previous experience within a health or social care setting, and either currently have, or be willing to work towards, NVQ level 2 in Health & Social Care or equivalent.
  • A full driving license and access to your own vehicle is required, as you will travel countywide within Northamptonshire.
